Overview

• Contract role in Alturas, California
• All shifts
• 36 hours per week
• 13-week contract

Required Qualifications

• Valid LPN / LVN license in California
• Skilled Nursing (1 year of experience)
• Skilled Nursing experience within the last year
• BLS

Role Details

• Works in the SNF service
• Reports to the DON
• Provides nursing care to acute patients under the direction of the charge RN
• May assist nursing staff in other departments when needed and qualified
• May work beyond normal hours and temporarily in other positions when necessary

Key Responsibilities

• Conducts individualized patient assessments and notifies the Charge Nurse and MD of abnormal findings
• Develops individualized plans of care with the healthcare team
• Performs ordered treatments and nursing procedures
• Performs assigned tasks such as EKGs, blood draws, and IV starts
• Provides patient and family education
• Administers medications and documents care
• Responds to emergencies and initiates resuscitation measures according to protocol
• Maintains confidentiality and provides non-discriminatory care
• Reports patient condition each shift
• Supports infection control and safety practices
• Assists with patient transfers, AM blood draws, specimen collection, orientation of new personnel, and staff education
• Participates in walking rounds, monthly staff meetings, annual CPR classes, and other in-service education
