Job Description

• Provide direct clinical social work services, counseling, discharge planning, and case management to hospital patients and their families.
• Assess and document emotional, social, financial, environmental, and follow-up referral needs.
• Collaborate with the interdisciplinary healthcare team to formulate and monitor psychosocial plans of care.
• Develop discharge plans and identify barriers to meeting estimated length of stay.
• Facilitate integration with community-based resources to optimize patient health.
• Support quality improvement, clinical research, and community networking efforts.
• Provide integrated mind-body-spirit care and patient advocacy.
• Interpret social, psychological, emotional, financial, and family issues for physicians and healthcare team members.
• Complete required documentation in medical records and hospital systems.
• Demonstrate ongoing professional skill enhancement and comply with hospital policies.

Requirements

• Specialty: Medical Social Worker - Hospital
• Contract Length: 8 weeks
• Education: Master of Social Work (MSW) from a CSWE-accredited school
• Licensure: ASW or higher license required; LCSW preferred (preferred but not mandatory)
• Experience:

  • Acute care experience required
  • Strong psychosocial assessment and critical thinking skills
  • Skilled in working with patients and families coping with loss and grief
  • Ability to work effectively with diverse populations (age, culture, ethnicity, gender)
• Skills:

  • Strong clinical decision-making and independent judgment
  • Excellent organizational, written, and oral communication skills
  • Knowledge of motivational, system, developmental theories, and group dynamics
  • Proficiency with word processing; database skills preferred
  • Ability to build and maintain constructive relationships with team members and external providers
• Physical Effort: Some standing, walking, stooping, bending, twisting, sitting, and computer use throughout the day; transportation available for referrals

Additional Notes

• Effective caregiving and advocacy skills for clients ranging from neonatal through geriatric populations are essential.
