Registered Nurse - Home Health (Travel)

Location: Crystal Falls, Michigan
Contract Length: 13 weeks

Job Details

• Shift: 8-hour day shifts (approximately 8:00 AM - 4:30 PM) with some flexibility depending on census
• Hours: 36 to 40 hours per week (flexible)
• On-Call:

  • Weekday call 1-2 nights as needed
  • Weekend and holiday call rotation
• Patient Load: 20-25 patients weekly, averaging 4-5 visits per day
• Travel:

  • Travel distances vary from 25 to 60 miles daily, with some days exceeding 100 miles due to large territory
  • Mileage reimbursement at current government rate

Requirements

• Experience: Minimum 2+ years of home health nursing experience
• Prior Travel Experience: Required
• Licenses & Certifications:

  • Active RN license in Michigan and Wisconsin
  • BLS certification
  • OASIS certification is a plus
  • Must have valid driver’s license
  • AHA or ARC certifications required by employer
• Charting Systems: Experience with OASIS and Epic required

Job Responsibilities

• Conduct OASIS admissions and manage all time points and episode management
• Coordinate care including physician calls and documentation in EMR in a timely manner
• Clinical competencies required in wound care, IV therapy, ports, lab draws, and chronic disease management
• Manage time effectively, adapt to change, and demonstrate dependability and accountability
• Maintain a positive attitude and compassionate approach

Additional Information

• Must live outside a 50-mile radius from the facility
• Flu and COVID vaccination required during flu season; mask policy enforced for unvaccinated travelers
• Eligible for 36 hours of unbillable low census hours during contract
• Call-back pay available for on-call work with a minimum of 2 hours

This position is ideal for experienced home health nurses with prior travel experience who are comfortable managing a large patient territory and flexible scheduling.
