Physical Therapist - Oregon

Job Details

  • Location: Dallas, Oregon
  • Contract Length: 13 weeks
  • Schedule: Monday to Friday, 40 hours per week guaranteed
  • Shift: AM
  • Workload: Average 4-6 visits per day
  • Coverage Area: Jackson County and surrounding territories within a 60-mile radius
  • Overtime: Must be pre-approved
  • Cancellation Notice: 2 weeks

Job Description

  • Provide home health physical therapy services
  • Productivity expectations: 28-32 points per week (points assigned based on visit types such as SOC, ROC, evaluations, routine visits, reassessments, and case conferences)
  • May be asked to assist in other nearby territories as needed

Required Qualifications

  • Valid Physical Therapist license in Oregon
  • Experience in a Home Health setting
  • Experience with Homecare Homebase (HCHB) documentation
  • Experience with OASIS
