Required Qualifications

• Valid RN license in North Carolina
• Associate of Science in Nursing / Associate Degree in Nursing
• Case Management (2 years of experience)
• Case Management experience within the last year
• Experience in an Acute Care setting
• Acute Hospital Case Management Experience
• Care Manager experience

Role Overview

• Provides ongoing patient assessment, planning, implementation, and evaluation to support care coordination, utilization review, and discharge planning
• Coordinates with the multidisciplinary team on discharge readiness, barriers to discharge, and transition planning
• Participates in care management meetings and follows up on action items
• Educates patients, families, and care teams on community resources and post-discharge support
• Documents assessment and plan of care in Epic
