Schedule

• 8 hours per day
• 32 hours per week minimum
• Monday through Friday, 8:00 AM to 4:40 PM
• On-call required 1 weekend every 1–2 months, based on need
• Weekend rotation and possible observed holiday availability may be required

Responsibilities

• Develops and revises the plan of care with the physician and other health care professionals
• Supervises home health aides and licensed practical/vocational nurses as allowed by state and federal regulations
• Completes comprehensive assessments, including OASIS and medication reconciliation
• Documents patient visits accurately and on time
• Performs initial and/or comprehensive nursing evaluation visits
• Communicates patient care updates, visit assignments, schedule changes, orders, and OASIS data sets to appropriate staff
• Coordinates care with physicians, patients, family members, and other clinicians
• Participates in meetings, case conferences, survey readiness, and performance improvement activities
• Assists with orientation of new staff and serves as a preceptor
• Adheres to the agency’s utilization management model

Required Qualifications

• Valid RN license in Arkansas
• Home Health experience within the last year
• BLS
• Associate of Science in Nursing / Associate Degree in Nursing
• Driver's License

Preferred Qualifications

• Home health experience
• OASIS experience
• HCHB experience

• Mileage reimbursement between visits is available
