• Coverage area includes Boston and surrounding regions

Schedule

• Monday through Friday, 8:00 AM to 4:30 PM
• 32-hour weekly guarantee
• On-call and weekend coverage required

Required Qualifications

• Valid RN license in Massachusetts
• Home Health (1 year of experience)
• Home Health experience within the last year
• Associate of Science in Nursing / Associate Degree in Nursing

Key Responsibilities

• Develops and revises plans of care with the physician and other health care professionals
• Supervises home health aides and licensed practical/vocational nurses as appropriate
• Provides clinical services within scope of practice and infection control standards
• Completes comprehensive assessments, including OASIS and medication reconciliation
• Documents patient visits accurately and on time

Preferred Experience / Skills

• Ability to work autonomously
• Quick learner with ability to onboard fast
• Home care experience, especially with post-acute and assisted living patients

• Mileage between visits is reimbursed
