Job Opportunity: Registered Nurse - Care Coordination (Case Management)

Contract Length: 13 weeks

Job Summary

Seeking an experienced Registered Nurse for a Care Coordination (Case Management) travel contract. The role involves acute hospital discharge planning, utilization review, and care coordination in a fast-paced hospital environment.

Required Qualifications

• Active California RN license
• BLS Certification
• Minimum 3 years of hospital nursing experience, including acute care coordination
• Proficient with Midas, InterCall, Cerner, Curaspan, and Microsoft Office programs
• Experience with discharge planning and utilization review
• Familiarity with placement agencies and Medicare/Medi-Cal guidelines related to SNF and lower-level placements
• Ability to manage socially complex discharges (e.g., conservatorship, under/uninsured patients)

Additional Requirements

• Must have prior acute care experience in California acute care hospitals (specific prior employer restrictions apply)
• Able to cover all adult hospital areas; no coverage of women’s health if no experience in that area
• Able to work 8-hour shifts with minimal time off requests; 12-hour night shifts possible if background includes Emergency Department experience
• No overtime available
• Must be adaptable, fast learner, flexible, with excellent attendance
• Professional business casual dress code (no open-toed shoes, minimal/no perfume)
• Must take scheduled lunches and breaks

Orientation & Training

• 4 days webinar on charting
• 2 days preceptor training

Work Environment

• Patient assignment ratio approximately 1:20
• Located in an underserved area with complex social discharge needs
• Staff work with room and board placements, conservatorships, and patients with limited resources

Shift Details

• Primarily 8-hour day shifts, every other weekend required
• Option for 12-hour night shifts with ED background
• Floating required across adult hospital units
