Job Opportunity: Case Manager (MSW or RN)

Location: Tuba City, Arizona
Employment Type: Contract
Contract Length: 13 weeks


Position Summary

This role serves as an Outpatient/Inpatient Case Manager, responsible for direct supervision and support of a specific group of high-risk patients. The Case Manager designs and manages a continuum of care aimed at empowering clients to achieve measurable outcomes and self-sufficiency. Responsibilities include assessment, service planning, resource acquisition, monitoring progress, and responding to emerging client needs.

The Case Manager will coordinate with community social service providers, healthcare providers, substance abuse, and mental health providers to meet client goals. Utilization review of patient cases in inpatient and outpatient departments is required, including reviewing medical necessity for referrals to specialty providers.

This position involves working closely with a multidisciplinary team to promote efficient and effective use of healthcare resources, focusing on whole-patient care to provide seamless continuity. Goals include quality care provision, reducing care fragmentation, enhancing patient self-care and quality of life, and cost containment.


Required Qualifications

Education

  • MSW Social Worker: Master’s Degree in Social Work (MSW)
  • RN Case Manager: Associate degree in Nursing, with a requirement to obtain a Bachelor’s degree in Nursing within 2 years of hire

License/Certification

  • Current Basic Life Support (BLS) certification by the American Heart Association (must be maintained throughout employment)
  • MSW Social Worker: Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) with a valid, unrestricted professional license in any U.S. state, territory, or commonwealth
  • RN Case Manager: Valid, unrestricted professional nursing license in any U.S. state, territory, or commonwealth

Experience

  • Minimum of 3 years of experience in a clinical care setting
  • Experience and knowledge in discharge planning, case management, or utilization review processes
  • Demonstrated knowledge of electronic health record (EHR) systems

Additional Information

  • Orientation occurs every 2 weeks
  • COVID & Flu vaccination exemptions may be accepted after submission of a declination form
  • Housing is shared contractor housing only; no pets or family allowed per policy
  • Fingerprinting must be submitted and cleared prior to start

This position offers a culturally rich environment serving Native American communities including Navajo, Hopi, and Southern Paiute tribes.
