Occupational Therapist - Rehabilitation Therapy
Location: Grand Junction, Colorado
Employment Type: Contract
Contract Length: 13 weeks
Job Description
Seeking a licensed Occupational Therapist (OT) to provide skilled, in-home occupational therapy services. The OT will deliver patient-centered, evidence-based care in patients' homes, supporting safety, functional independence, and quality of life through individualized treatment focused on activities of daily living and adaptive strategies.
Clinical Responsibilities
- Perform occupational therapy visits including evaluations, Start of Care (SOC), reassessments, recertifications, and discharge visits
- Develop and implement individualized plans of care based on patient needs, functional goals, and physician orders
- Provide skilled interventions such as ADL and IADL training, upper extremity strengthening, fine motor coordination, adaptive equipment training, energy conservation, and home safety/fall-prevention strategies
- Educate patients and caregivers on treatment plans, adaptive techniques, home programs, and safety strategies
- Communicate patient progress and changes to the interdisciplinary care team
- Collaborate in clinical oversight of Occupational Therapy Assistants (OTAs) when applicable and permitted by Colorado law and agency policy
Documentation & Systems
- Complete all required documentation accurately and timely in Homecare Homebase (HCHB)
- Ensure documentation complies with Medicare Conditions of Participation, payer requirements, and agency standards
- Maintain HIPAA compliance and privacy/security requirements
Care Coordination
- Collaborate with clinical staff including RNs, OTAs, PTs, STs, and leadership
- Participate in care coordination related to patient needs, visit completion, and regulatory requirements
- Provide OTA oversight when applicable and permitted
Required Qualifications
- Current, unrestricted Colorado Occupational Therapist license
- Graduation from an accredited Occupational Therapy program
- Valid driver’s license and reliable transportation
- Ability to practice independently in a home-based clinical setting
- Proficiency with or willingness to learn Homecare Homebase (HCHB)
Preferred Qualifications
- Home health experience
- Familiarity with Medicare documentation and home health regulations
- Experience working with geriatric and medically complex patient populations
