Occupational Therapist (Outpatient) - Travel/Contract Position
Location: Marion, Indiana
Specialty: Therapy/Rehabilitation

Contract Length: 13 weeks (approximately 3 months)
Start Date: January 5, 2025
Schedule: Monday-Friday, 8:00 AM - 4:30 PM (No weekends, evenings, or holidays)

Job Description
- Temporary need for an outpatient Occupational Therapist
- Work in an outpatient setting providing occupational therapy services
Requirements & Qualifications
Licensure & Certifications
- Valid State Occupational Therapy License (primary source online verification)
- BLS (Basic Life Support) - AHA only (Instructor, Provider, or RQI acceptable)
- ACLS (Advanced Cardiac Life Support) preferred but not required for this role
- Other certifications (PALS, NRP, EFM) not required for outpatient OT
Background Checks
- Criminal background check within 30 days of start (7-year search including SSN trace, counties, national criminal, and sex offender registries)
- EPLS/GSA/SAM and HHS/OIG checks must be done annually
Health & Immunization
- Physical exam within 1 year of start
- Fit mask test NOT required for outpatient physical therapy role (surgical mask acceptable)
- Tdap vaccine within the past 10 years or declination
- Negative TB test (2-step PPD, Qgold, or Tspot) within 3 months of start or clear chest X-ray if past positive
- Required immunizations or positive titers for:
- Rubeola (Measles)
- Mumps
- Rubella
- Varicella
- Hepatitis B (titer or declination accepted; vaccination alone not accepted)
- Drug screening (5 panel or more) within 30 days of start
- Seasonal flu vaccination required during flu season (Oct-April) unless approved exemption
Competencies & Facility Requirements
- Medication test/pharmacology competency exam as applicable
- Annual skills checklist required
- Completion of annual mandatory facility-specific training (online, approx. 40 minutes)
- Compliance with facility dress code and IT access requirements
References
- Two professional references within 3 years of start date
Additional Notes
- This is a travel/contract position with a defined 13-week duration
- No weekend, evening, or holiday shifts required
- Facility mandates annual background and health document renewals as applicable
- Fit mask testing is generally not required for this outpatient OT role
