Required Credentials & Documentation

Identification

• Current and valid Driver’s License, government photo ID, or passport
• Proof of name change if applicable (certifications/licenses must have current legal name)

Background Checks (Required within 30 days of start)

• 7-year background check (renew annually) including:

  • Social Security Trace
  • Nationwide, Statewide, and County criminal searches
  • National Sex Offender Registry
  • OFAC, FACIS L1
• EPLS/GSA/SAM check
• HHS/OIG check
• Education verification (3rd party verification of highest education level)

Certifications (AHA only, certificates of completion NOT accepted)

• ACLS (Advanced Cardiac Life Support) – required for ER, PCU, CVICU, ICU, Cath Lab, LAD, IR, Urgent Care
• BLS (Basic Life Support)
• PALS (Pediatric Advanced Life Support) – required for ER, NICU, PICU, Pediatrics, Interventional Radiology, Urgent Care
• NRP (Neonatal Resuscitation Procedure) – required for Nursery, NICU, L&D, Mother/Baby
• TNCC (Trauma Nursing Core Course) – required for ER
• EFM (Electronic Fetal Monitoring) – AWHONN Intermediate required for L&D (Advanced accepted)
• Technical Certification Verification (as applicable):

  • Certified OR Tech/Surgical Tech (CST) - NBSTSA or NCCT
  • Radiology Tech (Generalist, Cath Lab, Interventional Radiology) - ARRT
  • Phlebotomist - National Tech Cert (ACSP, AMT, NAHP, NHA preferred)
• State License Verification (required within 30 days of start)

  • Radiology Techs require Montana State License (temp licenses NOT accepted)
• NURSYS Verification for RNs and LPN/LVNs (required within 30 days)
• CPI (Crisis Prevention Institute) preferred for Psych

Competencies

• Dysrhythmia/EKG Competency Exam preferred for Tele, Stepdown, PCU, ICU, ER (80% pass rate)

Health & Safety Requirements

Physical & Health Documentation

• Physical exam within 1 year of start, signed by MD, NP, or PA (must state good health, no communicable diseases, no physical limitations)
• Quantitative fit mask test with 3M mask within 90 days prior to start (qualitative tests not accepted, must be clean-shaven, renewed annually)
• Tetanus/Diphtheria/Tdap vaccine within 10 years or signed declination (TD not accepted)
• Negative TB test or chest X-ray, renewed annually (various accepted testing options)
• TB questionnaire for past positive TB, renewed annually
• Immunizations or positive IGG titers required for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (or declination within last year)
  • Varicella
• Negative drug screen within 30 days of start, renewed annually (9-panel plus additional substances)
• Seasonal Flu vaccination/documentation required Oct 31 - Mar 31 (declinations accepted with proper documentation)
• COVID-19 vaccination or medical/religious exemption required

Employment History & References

• 2 references required, including a direct supervisor reference within the last year (letters accepted)
• Current employment application covering 7 years with explanations for gaps over 30 days
• Employment verification preferred for last 5 years
• Skills checklist required as applicable
