Location: Toledo, Ohio
Facility Type: Level I Trauma Center, largest acute care hospital in Toledo metro area
Unit: Observation Unit (Mixed PCU, Med Surg Telemetry, Med Surg)


Job Details

  • Patient Diagnoses Include:
    • Chest pain, Abdominal pain, Atrial Fibrillation, TIA
    • Anemia/GI Bleed, Dehydration/AKI, Syncope, Vertigo
    • COPD, Asthma, UTI/Pyelonephritis, Headache/Migraine
    • CHF, Seizure, Carbon Monoxide Poisoning, Pancreatitis, Cellulitis
    • Post-outpatient surgery complications
  • Unit Details:
    • Fast-paced observation unit with length of stay goal 18-24 hours
    • Approximately 2/3 of patients managed under ED observation providers (7am-7pm)
    • Telemetry monitoring required
  • Ratios: 1:4 nurse-to-patient ratio on all shifts
  • Experience Required: Minimum 2 years
  • Skills Required:
    • Familiarity with intermediate medication drips (heparin, nitro, cardizem, insulin, etc.)
    • Charge nurse experience required
    • Observation medicine experience preferred
  • Certifications Required:
    • ACLS (AHA provider, instructor, or RQI accepted)
    • BCLS (AHA provider, instructor, or RQI accepted)
    • NIH Stroke Certification (updated annually unless 2-year expiration)
  • Schedule: Weekend shifts required every other weekend
  • Floating: Yes, within service line and between multiple facilities (Neuro/Stroke, Cardiac, Ortho, Trauma, Oncology, General Surgery, Uro/Vascular, Observation)
  • Block Scheduling: Not accommodated
  • Local Candidates: Not accepted within 50 miles radius
  • Pending License: Not accepted
  • Return to Office (RTO) Restrictions: Greater than 7 days off requires management approval

  • Facility: Level 1 trauma center, teaching hospital

Compliance & Documentation Requirements

  • Background Checks:
    • 7-year background check within 12 months of start, updated annually
    • EPLS/GSA/SAM and HHS/OIG checks required annually
    • Fingerprinting (BCI and FBI) required within 12 months for certain units; updated annually
  • Education Verification: Required for all candidates
  • Competency Exams:
    • Medication/Pharmacology competency as applicable
    • Dysrhythmia/EKG competency exam required annually for RNs (pass/fail, 2 attempts allowed)
    • Conscious Sedation competency for ER & ICU if applicable
  • Health Documents:
    • Physical exam within last 12 months (latex-free PPE available)
    • N95 Fit Test within 12 months, updated annually (KN95 not accepted)
    • Immunizations: Tdap (within 10 years), Measles, Mumps, Rubella (MMR titers or vaccines or approved exemptions), Varicella, Hepatitis B (series, titer, or declination)
    • Negative TB test (PPD, QGold, or TSPOT) within 3 months, updated annually
    • Seasonal Flu vaccination or approved exemption required during flu season
    • Negative 10-panel drug screen within 12 months (instant acceptable unless on Rx)
  • References: Two professional references required within 2 years, including at least one from a leader (Charge RN or above)
  • Skills Checklist: Required for all candidates

  • Floating may require working across multiple units and facilities within the system.
  • Fast-paced environment with a focus on observation medicine and telemetry.
  • Candidates must be prepared for weekend shifts every other weekend.
  • Strict compliance with certifications, background checks, and health documentation is mandatory.
  • No local candidates within 50 miles accepted to ensure travel status.
