Job Details

• Experience Required: Minimum 1 year recent experience in Physical Therapy, Home Health specialty
• Prior Travel Experience: Not accepted for first-time travelers (First Time Travelers: NO)
• Average Patients per Day: 6-7
• Guaranteed Hours: 40 hours per week
• Types of Cases: Geriatric patients
• Territory: Up to a 60-mile radius from the hub/office

Compliance & Background Requirements

• Background checks required within 90 days of start, including:

  • SSN Trace
  • County searches for last 7 years of residence/work/school
  • FACIS Level 1M
  • Nationwide Sex Offender
  • Motor Vehicle Records (MVR)
  • Education Verification
  • Work History Verification (7 years, no gaps)
  • State-specific searches as applicable (e.g., Ohio BCI and FBI fingerprinting updated annually)
• Fingerprinting required (Ohio BCI and FBI prints must be current within 12 months; reprints required if candidate has lived/worked outside Ohio since last prints)
• EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
• Education verification of highest degree related to profession required
• References: 2 professional references within 2 years of start
• Employment application must include full 7 years of work history with no gaps

Certifications & Competencies

• BLS (Basic Life Support) certification required (AHA, ARC, Military accepted)
• Orientation Safety Manual Acknowledgement (updated annually)
• State License verification (primary source within 90 days of start and upon expiration)
• NURSYS verification (for applicable professions)
• Medication Test/Pharmacology Competency Exam (as applicable)
• Competency Exam (as applicable to specialty)

Health & Immunization Requirements

• Physical exam within last 12 months (not required annually)
• Negative TB test or chest X-ray within required timeframes
• Annual TB questionnaire required
• Immunizations or titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Drug testing: 9-panel drug screen within last 12 months (instant acceptable)
• Flu seasonal vaccination or facility-specific declination form required during flu season
• COVID vaccination or facility-specific declination required

Facility Specific Requirements

• Proof of current auto insurance required (for all Home Health positions involving driving)
• Completion of Acceptable Use Policy prior to start
• Flu and COVID statement form required if declining flu vaccine
• Agency background attestation required before start

Summary

This is a travel Physical Therapist position specializing in Home Health care with a focus on geriatric patients. Candidates must have at least 1 year of recent experience in home health PT, OASIS and HCHB EMR experience, and BLS certification. First-time travelers are not accepted. The role requires strong documentation and communication skills, adherence to dress code, and compliance with extensive background and health requirements. The contract length is approximately 13 weeks.
