Location / Coverage

• Based in San Ramon, California
• Home Health coverage areas:

  • Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City
  • Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont
• May visit up to 6 patients per day
• Occasional maximum radius of 50+ miles
• Traveler is expected to have their own transportation

Required Qualifications

• Valid Physical Therapist license in California
• BLS issued by American Heart Association or American Red Cross

Preferred Qualifications

• 1 year of Home Health experience

Schedule / Work Expectations

• 1:1 ratio
• 27 productivity points standard
• Must see 5–6 patients per day
• Guaranteed hours do not apply in the first 2 weeks for ramp, holiday weeks, and time off weeks
• Weekends may be used to meet productivity
• Timely communication and documentation required
