Job Summary

Seeking an Occupational Therapist specializing in Home Health therapy. This role involves providing care primarily to geriatric patients within a 60-mile radius of the office hub. The position requires flexibility with travel territory and timely completion of documentation.

Required Experience & Qualifications

• Valid Occupational Therapist license in Washington
• Minimum 1 year of recent Occupational Therapist experience, including 1 year in Home Health
• Basic Life Support (BLS) certification
• Experience with Homecare Homebase electronic medical record system

Required Systems Experience

• OASIS experience required
• Experience with Home Care Home Base (HCHB) electronic medical record system required

Background & Compliance Requirements

• Background checks including SSN trace, county searches, FACIS Level 1, nationwide sex offender, motor vehicle records, education verification, and work history verification (as applicable) must be current within 90 days of start and updated annually.
• State-specific background checks and fingerprinting requirements apply, including on-site fingerprinting for candidates who have not lived in Washington for the past 3 years.
• Physical exam within the last 12 months required (not annually).
• Fit mask test (N95 accepted) required.
• Negative TB test or chest X-ray within 1 year of start required; TB questionnaire required annually.
• Immunizations or proof of immunity required for Measles, Mumps, Rubella, Hepatitis B, and Varicella.
• Drug screening (9 panel) within 12 months required.
• Seasonal flu vaccination or facility-specific declination required.
• COVID vaccination or facility-specific declination required.

Additional Requirements

• Proof of current auto insurance required due to driving responsibilities.
• Must complete orientation safety manual acknowledgment annually.
• Must provide 2 professional references within 2 years of start.
• Full 7 years of work history required on application with no gaps.
• Annual skills checklist and competency exams as applicable.

• Traveler must be flexible with territory assignments.
• Cell phone use is not permitted during work hours.
• All compliance and credentialing documents must be maintained and up to date prior to start.
