Job Opportunity: Physical Therapist - Home Health

Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

• Specialty: Home Health Physical Therapy
• Territory: West Team covering Livermore, Pleasanton, Dublin area (up to a 60-mile radius from the hub/office)
• Average Patients per Day: 5-7
• Guaranteed Hours: 40 hours per week (must meet productivity points to qualify; holidays and RTO not included)
• Types of Cases: Geriatric
• First-time travelers accepted
• Dress Code: Scrubs (any color), no jeans
• Device Provided: iPad
• Mileage reimbursed at IRS standard rate
• No on-call or weekend work required
• Flexibility with territory expected

Required Qualifications

• Valid Physical Therapist license in California
• Minimum 1 year of recent experience as a Physical Therapist in Home Health
• Basic Life Support (BLS) certification required (American Heart Association or American Red Cross preferred; online courses not accepted)
• Experience with Homecare Homebase (HCHB)

Required Documentation & Compliance

• Government-issued photo ID (Driver’s License or State ID)
• Background checks including federal, county, sex offender, motor vehicle record, and FACIS Level 3 (within 30 days to 1 year depending on check)
• Education verification (diplomas/transcripts or third-party verification)
• Physical exam within 6 months prior to start
• Negative TB test or chest X-ray within 6 months (chest X-ray accepted up to 5 years if history of positive TB)
• Immunization or titer proof for Measles, Mumps, Rubella (for candidates born in 1957 or later)
• Hepatitis B vaccination series, titer, or declination
• Varicella immunity proof (titer, vaccination, history of chickenpox, or declination)
• Drug screening: 9 or 10 panel drug screen within 30 days to 1 year of start
• Seasonal flu vaccination or declination during flu season (October 1 - March 31)
• Professional references: 2 for travel/allied/per diem acute care; 1 for per diem non-acute care (within 3 years)
• Annual skills checklist and competency exams as applicable

Additional Notes

• Locals are accepted and paid at travel rates
• No cell phone use required for the job
• Flexibility with travel territory is expected
