Job Opportunity: Step Down Nurse (LVN / LPN)

Job Description

• Work in a Progressive Care Unit (Step Down) with 3 shifts of 12 hours each (Day shift: 7:00 AM – 7:30 PM)
• Must have experience with EPIC electronic health record system
• Floating required: may be assigned to various units including High Risk Respiratory Unit/COVID and other specialty clusters at multiple nearby campuses (within 15 miles)
• Must be available to work every other weekend and holidays
• Employer reserves the right to cancel up to 2 shifts during the contract due to low census and may cancel the contract with 2 weeks' notice without penalty

Required Qualifications

• Valid Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) license in Ohio
• Minimum of 1 year of Long Term Care experience within the last year
• Experience with EPIC electronic health record system
• Telemetry experience
• Basic Life Support (BLS) certification issued by the American Heart Association
• Ability to float to different units as assigned
• Must be able to work 3 consecutive 12-hour day shifts

Certifications & Competencies

• Basic Life Support (BLS) - AHA certification required
• Advanced Cardiac Life Support (ACLS) - Not required for LPNs
• Dysrhythmia/EKG Competency Exam: Must be passed within 1 year prior to start or completed within 72 hours of contract signing (3 attempts allowed, 80% passing score)
• Medication/Pharmacology Competency Exam within 2 years of start
• Other certifications (e.g., PALS, NRP, TNCC, NIH Stroke Certification) are not required for this LPN role

Documentation & Compliance Requirements

• Government-issued photo ID (Driver’s License or State ID)
• Ohio licensure primary source verification
• Background checks including:

  • 7-year criminal background check (all counties/states)
  • EPLS/GSA/SAM and HHS/OIG checks
• Education verification (Primary source)
• Physical exam within 2 years indicating fitness for work and free of communicable diseases
• Negative TB test or chest X-ray (annual update if past positive TB)
• Immunizations or proof of immunity for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Varicella
  • Hepatitis B vaccination series, immune titer, or declination
• Seasonal flu vaccination or approved medical/religious exemption required during flu season
• Drug screening: Negative 9 or 10 panel drug screen including marijuana within 12 months prior to documentation due date; lab work must be completed within 72 hours of contract signing
• Respiratory safety clearance within 1 year of start, renewed annually
• Medical questionnaire for respiratory protection must be submitted prior to orientation
• Skills checklist updated within 1 year prior to documentation due date
• Two professional references within 2 years (one must be a manager/supervisor/charge nurse) or employment verification if references unavailable
• Current resume or employment application required

Additional Notes

• Travelers are first to float at employer’s discretion and may be assigned to support nursing care teams in various units
• Travelers are not eligible for employee incentives
• Fingerprinting is not required for this assignment
• Must comply with all facility-specific documentation and credentialing requirements prior to start

This position is ideal for LVN/LPN nurses with EPIC experience who are flexible with floating and available for a 13-week travel contract on day shifts in a progressive care setting.
