Physical Therapist - Home Health (Travel/Contract)

Location: Camden, South Carolina
Contract Length: 13 weeks
Shift: Day shift, Monday to Friday, 8:00 AM – 5:00 PM (40 hours/week)
Patient Visits: 5-10 patients per day, primarily home visits
Travel: Must be willing to travel up to 75 miles from home clinic; average travel 30-40 miles/day
Work Setting: Home health only (no facility visits)

Job Description

• Provide physical therapy services in patients' homes, including start of care and follow-up visits.
• Participate in periodic office meetings.
• Productivity expectation: 30 points per week (points assigned based on visit type and activities such as documentation and travel).
• No guaranteed hours; if patient census is low, floating to nearby clinics may be discussed.
• On-call requirements vary by location; typically none for this position.
• Local candidates accepted at the same rate.

Required Qualifications

• Valid Physical Therapist license in South Carolina
• Minimum 1 year of Physical Therapist experience within the last year
• Experience working with adult patients
• Experience in a home health setting
• BLS certification

Documentation & Compliance Requirements

• Must complete a standard background check covering the last 7 years, including SS trace, county, nationwide, sex offender, and state-specific searches.
• Drug screening: 10-panel drug test required.
• Physical exam within 12 months prior to start, confirming good health and no communicable diseases.
• Negative TB test or chest X-ray within 12 months, updated annually.
• Immunization or titer proof for Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella, and COVID-19 vaccination or declination.
• Seasonal flu vaccination or documented declination required.
• Education verification for degree applicable to physical therapy.
• Two professional references required.
• Verification of work history for at least the last 7 years or two most recent employers.
• Must review and sign various compliance documents prior to start, including confidentiality, HIPAA, privacy policies, and home health-specific scenarios and policies.

Additional Notes

• Mileage reimbursed at IRS standard rate; compensation is from patient home to patient home (commute to/from first and last patient is not billable).
• Weekend work requirements vary by location and should be discussed during the interview.
• Contract may lead to permanent hire if possible.
• EMR systems used include HomeCare HomeBase, OASIS, and Outlook.
