Case Manager (Registered Nurse) - Travel/Contract

Location: Williamsburg, Virginia
Contract Length: 13 weeks
Shift: Day, 10:00 AM – 6:00 PM (5 days/week, 8 hours/day)

Job Description

• Home hospice case manager role
• Must have experience specifically with home hospice case management (no exceptions)
• Patient census approximately 35-45 patients
• Basic Life Support (BLS) certification required

Required Qualifications

• Valid Registered Nurse (RN) license in Virginia
• Minimum of 1 year of case management experience, including experience within the last year
• BLS certification issued by the American Heart Association or American Red Cross
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Experience in a Home Health setting

Competencies & Testing

• Medication/Pharmacology competency exam (as applicable)
• Other competency exams as applicable to specialty
• Pass/fail testing (e.g., EKG/medication) administered during orientation
• Annual skills checklist update required

Facility-Specific Requirements

• Photo for badge meeting specific guidelines (recent, professional, neutral background, no hats/headphones, etc.)
• Signed confidentiality agreement
• Review and sign scrub color, safety brochure, dress code, drug-free policy, and hand washing hygiene policies prior to start
• Annual sworn disclosure for hospice/home health positions
• Tuberculosis questionnaire updated annually

Additional Notes

• Must be minimum 21 years old to drive; 25 if transporting patients
• Prior travel experience is not explicitly required for this position
• No Advanced Cardiac Life Support (ACLS) or other certifications specified beyond BLS for this role
