Job Summary

Seeking a Physical Therapist specializing in Home Health with a minimum of 1 year of recent experience. The role involves managing an average of 5-7 geriatric patients per day, completing 6 visits daily or a minimum of 30 points weekly, with guaranteed 40 hours per week.

Key Responsibilities

• Provide physical therapy services to geriatric patients in a home health setting.
• Complete timely communication and documentation.
• Maintain a caseload within a 60-mile radius from the office hub.

Required Qualifications

• Valid Physical Therapist license in California.
• Minimum 1 year of recent experience in Physical Therapy Home Health.
• Basic Life Support (BLS) certification.
• Experience with OASIS in a Home Health setting.

Work Environment

• Average patient load: 5-7 patients per day.
• Travel within a 60-mile radius.
• Locals accepted at travel rate.
• No on-call required.

Compliance and Documentation Requirements

• Background checks including SSN trace, county searches, FACIS, nationwide sex offender, motor vehicle records, education and work history verification.
• State-specific background and fingerprinting requirements as applicable.
• Verification of highest degree related to profession.
• Annual acknowledgment of safety manual and orientation.
• Primary source verification of state license within 90 days of start and upon expiration.
• Competency exams as applicable.
• Proof of current auto insurance required due to driving responsibilities.
• Physical exam within last 12 months.
• Fit test for N95 mask required.
• Negative TB test or chest X-ray within 1 year; TB questionnaire annually.
• Required immunizations or proof of immunity: Measles, Mumps, Rubella, Hepatitis B, Varicella.
• 9-panel drug screen within last 12 months.
• Seasonal flu vaccination or facility-specific declination form required.
• COVID vaccination or facility-specific declination required.
• Two professional references within 2 years of start.
• Full 7 years of work history documented with no gaps.

Additional Notes

• Timely communication and documentation are critical.
• Mileage reimbursed at IRS standard rate.
• Dress code strictly scrubs (any color), no jeans.
• iPad provided for documentation; cell phone use not allowed during visits.
