Registered Nurse – Long-Term Care (Travel/Contract)
Location: Vestal, New York
Contract Length: 16 weeks
Job Details
- Shift: Evening, 8-hour shifts (14:45–23:15), 5 days per week
- Weekend Requirement: Every other weekend required
- Facility Size: 300 beds
- Patient Population: Alzheimer's & Dementia patients
- Staff Ratios: Approximately 1 nurse to 18-20 residents
- Schedule: Cannot change shift or add RTO once offered; RTO only approved at time of submission
- Attendance: Strong attendance and punctuality required; call-outs must be given with more than 24 hours’ notice
- Locals Accepted: Yes
Required Qualifications
- Valid RN license in New York
- Associate of Science in Nursing / Associate Degree in Nursing
- Minimum 1 year of Long Term Care experience within the last year
- BLS certification (accepted from any recognized entity; online-only courses acceptable)
- Experience in a Long Term Care setting
Responsibilities
- Administer medications and treatments as prescribed
- Perform medication passes accurately and safely
- Monitor and report changes in resident condition
- Complete timely and accurate documentation
- Provide nursing care according to facility policies
- Supervise CNAs on assigned units

Orientation & Training
- Start dates every 2 weeks due to orientation schedule
- Must complete facility-required Relias modules prior to assignment start date (pass grade of 80% on modules; 90% on medication administration exam)
- Additional modules may be assigned after start date and must be completed by assigned due dates
Facility & Work Environment
- Type: State/Government-run Long-Term Care Center with rehabilitation therapies, skilled nursing, memory care, wound care, and more
- Attire: Any solid color scrubs and closed-toe shoes
- Charting system: PCC (Point Click Care) – no submission required
- Parking: Free
- Mileage reimbursement: None
Compliance & Documentation Requirements
- Government-issued photo ID (front copy)
- Background checks including 7-year history, EPLS/GSA/SAM, and HHS/OIG (updated annually)
- Physical exam within 1 year of start, clearing candidate physically and for communicable diseases
- Negative TB test or chest X-ray within 1 year (or TB questionnaire if positive history)
- Immunizations or proof of immunity for Measles, Mumps, Rubella (boosters if titers low)
- Flu vaccine or declination on file year-round (mask required if declined during flu season)
- COVID-19 vaccination or declination required
- Pneumonia vaccination proof or declination required
- Drug test (9-panel) required for direct hires within 1 year of start (not required annually)
- Harassment prevention training certificate required for subcontractors
- References: 2 professional references within 2 years of start preferred (supervisors preferred but not required)
- Resume/application required
- New York NYSIIS Consent Form required
Additional Notes
- Must be able to provide proof of license and certifications at submission
- No shift changes or additional time off allowed after offer acceptance
- Modules access sent via email within 48 hours of offer acceptance; check spam/junk folders
- Medication administration exam must be passed with 90%; unlimited retakes allowed
