Job Overview
- Provide continuous observation of residents, noting changes in mental and physical well-being and communicating with supervisors and physicians as needed.
- Receive and give detailed shift reports on resident status and events.
- Accurately transcribe and carry out physician’s orders, including medication administration and treatments, with proper electronic documentation.
- Supervise Licensed Nursing Assistants (LNAs) according to New Hampshire Board of Nursing requirements.
- Assign and oversee nursing staff resident care consistent with their abilities and scope of practice.
- Assist direct-care staff with nursing care as time allows.
- Accompany physicians on resident rounds to address medical needs and facilitate communication during 60-day reviews.
- Administer oxygen, gastrostomy/jejunostomy feedings, and perform wound care including knowledge of wound vacs.
- Monitor unit activities such as therapy appointments, ambulation lists, vital signs, and nutritional documentation.
- Oversee infection control practices and maintain a clean, hazard-free unit environment.
- Admit, discharge, and transfer residents with appropriate documentation and notifications.
- Complete 24-hour reports, check new orders, and maintain communication with Nurse Manager/Nursing Supervisor regarding resident and staff issues.
- Communicate with residents’ families to resolve concerns, notify of new orders, and obtain consent when necessary.
- Manage assigned unit(s) during shift under direction of Nursing Supervisor or Director of Nursing Services.
- Follow state and federal regulations for controlled medication storage, usage, and documentation, including shift change counts.
- Assist with orientation and ongoing education of nursing staff; evaluate staff performance and recommend disciplinary action as needed.
Required Qualifications
- Minimum 3 years of current LVN/LPN experience within the last 6 months.
- IV certification required; must be willing to obtain during onboarding if not already certified.
- Must be willing to float to other units where experienced.
- COVID-19 vaccination required; no declinations or exemptions accepted. Proof of vaccination must be provided.
- Locals residing within 50 miles of the facility are not eligible for this assignment.
- Previous travel experience is highly preferred.
- Experience in rural or small facility settings is highly preferred.
Additional Information
- Candidates must have their own transportation due to the rural location with limited housing and no public transportation.
- Dress code: Matching scrubs (top and bottoms).
- Orientation hours: 0 hours (no paid orientation).
- Overtime and callback pay policies apply (details withheld).
- Cancellation policy allows up to 3 shift cancellations per 13-week contract.
- Annual seasonal flu vaccine required or declination with mask-wearing during flu season.
- Timecard process requires clocking in/out at the facility; no paper timecards signed by managers.

- The facility provides 24-hour nursing care for residents needing intermediate or skilled nursing care.
- Emphasis on preventive and restorative care addressing physical, emotional, spiritual, and psychological needs.
- Care plans are developed collaboratively with residents and their families or legal representatives to ensure a comfortable, home-like environment.
- Medicaid licensed Intermediate Care Facility (ICF) and Medicare approved Skilled Nursing Facility (SNF).
