• Varicella vaccination or titers
• Annual TB Questionnaire (within past 12 months)
• Physical and Vision Exam within past 12 months (must include systems check)
• 12 panel drug screen (NO THC) within past 12 months

Background & Professional Checks

• 7-year Criminal County Search, SSN Trace, National Criminal Database (within 12 months)
• State instant background accepted if county results delayed
• National Sex Offender Registry check (within 30 days prior to start)
• State Sex Offender Registry check (within 30 days prior to start)
