Overview

• Location: Newton, Massachusetts
• Employment type: LTA
• Contract length: 13 weeks
• Shift: 08:00 - 16:30
• Weekend rotations are once every 6 weeks
• Ability to travel to cases as assigned

Required Qualifications

• Valid Physical Therapist license in Massachusetts
• Physical Therapist (1 year of experience)
• Physical Therapist experience within the last year
• Experience in a(n) Home Health setting
• 1 year of case management experience

Job Summary

• Provide physical therapy services to patients in their place of residence
• Patients may range in age from birth to over 100 years
• Collaborate with patient, family, physician, and other allied health professionals
• Use timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and standards of practice

Onboarding / Compliance

• Resume with past 7 years of work experience
• 2 references within 4 years of submission, including at least 1 supervisor or above in skillset
• Skills checklist completed within 1 year of submission
• MVR check required for all home care positions
• Primary source verification of professional license and/or certification
• N95 fit test within 365 days of start date
• Respirator Medical Clearance attestation if fit test is required for the skill
• Nurse Aide Registry check
• MMR documentation or proof of immunity
• Influenza vaccine
• TB test documentation
• Health statement signed by MD/DO/NP/PA
• Criminal background check
• Massachusetts CORI
• SSN trace
• National Sex Offender Check
• Exclusion checks
• Education verification
• Confirmation of Assignment/POI form
• Beeline attestation

• Positive PPD with negative chest X-ray documentation
• Name change documentation
• TB symptom survey if positive PPD
