• Two recent references (minimum one supervisor or above)
• Skills checklist required for submission
• FEMA ICS 100 Training must be completed (no exceptions)
• Respirator Medical Clearance Attestation if fit testing is required
• Education verification of highest level achieved
• Social Security Number trace for past 7 years
• National sex offender check within 30 days of start and updated annually
• Exclusion checks (OIG, GSA, MassHealth, NH Medicaid) within 30 days of start and annually

Additional Information

• MA Conditional Licenses are not accepted
• Flu mist vaccine must be administered by a provider or pharmacy; self-administered flu mist is not accepted
• TB screening form and chest x-ray documentation required if positive TB test
• Background checks and credential verifications must be updated if there is a break in assignment greater than 6 months
