- Week 1: Monday to Friday, 12 PM – 8:30 PM
- Week 2: Monday, Tuesday, Wednesday, Friday 12 PM – 8:30 PM and weekend 3 PM – 11:30 PM (every other weekend scheduled)
- On-call assistance preferred

Current license/certification verification dated within 30 days of start.
Fit test and respirator evaluation with physician signature.
COVID-19 vaccine proof (including booster through September 2025), or approved exemption/declination (declination not accepted in CA or IL).
Proof of MMR vaccination (2 doses or IgG titer).
Hepatitis B vaccination series initiated prior to start (titers not accepted).
Influenza vaccine proof or declination (optional).
2-step PPD or IGRA TB test; if positive, chest X-ray within last year (for CT and certain states).
Varicella vaccination proof or IgG titer.
Physical exam within the last year confirming fitness to work.
10-panel drug screen completed within 30 days of start (no THC).
Completion of skills checklist provided by agency.
Signed professional forms including code of conduct, confidentiality agreement, conflict of interest disclosure, integrity and audit services form, and patient information security agreement.
Background checks including federal, nationwide, county criminal (last 7 years), SSN verification, EPLS, OIG, NSO, OFAC, and education verification.
CORE exam within the last year.
System access form completed and uploaded one week before start.

All documentation must be uploaded to the credentialing system prior to or at onboarding.
Weekend and on-call availability is preferred.
This position involves direct patient care in a hospital setting.
