BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel. Must provide front and back images of the card.
License/Certificate Verification: Must be dated within 30 days of start. Online verification with machine-generated date required.

Fit Test
10 Panel Drug Screen: Completed within 30 days of start (preferred within 14 days). Must include specific substances; no THC allowed.
COVID-19 Vaccine: Proof of booster by Sept 2025, or declination/exemption with agency letterhead (declination not accepted in CA or IL).
Influenza Vaccine: Proof of vaccine, declination, or exemption for current flu season (declination not accepted in CA or IL).
MMR: Proof of 2 vaccinations or IgG titer lab report.
Hepatitis B: Must have started vaccination series prior to start; titers not accepted.
Varicella: Proof of 1 vaccination or IgG titer lab report.
2 Step PPD OR IGRA (Quantiferon/Tspot): TB testing required; positive TB requires chest x-ray per regional guidelines.
Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
