Job Opportunity: RN Medical/Surgical - Travel Position

Location: Cleveland, Ohio
Contract Length: 13 weeks

Job Description

• Every Other Week (EOW) / Every Other Holiday (EOH) required
• Intermittent charge nurse responsibilities may be required
• Charge training and shadowing offered

Required Certifications & Licenses

• BLS (American Heart Association only) – Required for clinical positions
• Valid Permanent RN License – Temporary licenses not accepted; must have primary source verification within 30 days of start and updated every 2 years with license renewal
• UHS Ohio Fingerprints – Ohio BCI&I check and FCRA-compliant National Database Criminal background check or FBI fingerprint check depending on residency duration
• UHS Education Verification – Highest level of education verified; international education requires equivalency documentation
• UHS Employment Verification – Verification of employment within last 5 years through third-party verification

Health & Immunization Requirements

• COVID-19 Vaccine – Proof preferred but not required; medical or religious exemptions accepted with approval
• MMR (Measles, Mumps, Rubella) – Positive titers within last 15 years required; booster protocol applies if low titer
• Varicella – Positive titers within last 15 years required; booster protocol applies if low titer
• Hepatitis B – Proof of full series, partial series, or signed declination accepted; positive titers accepted within 15 years
• Tdap – Proof of vaccination within past 10 years; declinations not accepted
• Influenza Vaccine – Required during flu season (Oct-Mar); medical or religious exemptions require signed forms and physician statement
• 2 Step PPD OR IGRA (Quantiferon/Tspot) – Negative test within last 6 months; if positive, clear chest x-ray and current TB questionnaire required
• Positive PPD – If positive, chest x-ray within 5 years and current questionnaire required
• Fit Test – Completed within 1 year of start; specific approved mask models required
• Physical/Health Statement – Pre-employment physical exam within 1 year stating ability to work without restrictions

Background & Screening Requirements

• Criminal Background Check – Completed within 6 months prior to start; includes SSN trace, county searches, national criminal sweep, and NSO
• UHS Social Security Trace – If not included in background check, completed within 1 year prior to start
• UHS NSO (National Sex Offender) – If not included in background check, completed within 30 days prior to start
• UHS OIG Search – Completed within 6 months prior to start; not annual
• UHS SAM/EPLS/GSA/OFAC Search – Completed within 6 months prior to start; not annual
• Drug Screen – 10-panel drug screen within 60 days of start; instant drug screens not accepted; must include lab report and MRO if positive
• UHS Module Completion Certificate – Safety and compliance training modules must be completed prior to clearance

Competency & Skills

• UHS Competency Exam – Must score at least 80%; completed within 1 year prior to start
• UHS Core Competencies – Includes HIPAA, Joint Commission-NPSG, OSHA topics; 80% passing score required; completed within 1 year prior to start
• UHS Skills Checklist – Clinical skill-specific checklist completed within 1 year prior to start; required upon initial hire only

Additional Requirements

• Driver's License/State ID – Non-expired photo ID required
• UHS Submission Packet – Resume including education and work history, 2 professional references within last 2 years, and completed job application (agency form)
• UHS Badge Photo – Headshot with plain background, shoulders up, JPEG format
