Required Qualifications

• Valid Clinical Lab Scientist (CLS) license in Nevada
• Minimum of 1 year of Clinical Lab Scientist (CLS) experience
• Clinical Lab Scientist (CLS) experience within the last year

Health and Immunization Requirements

• COVID-19 vaccination: 2 vaccines plus booster or completed declination
• MMR: Proof of 2 vaccines or positive titer showing immunity
• Hepatitis B vaccination (declinations not accepted)
• Influenza vaccine or declination required during flu season (declinations not accepted)
• Tuberculosis test within one year of start date
• Varicella vaccination or titer showing immunity
• Tdap vaccination (declinations not accepted)
• If positive PPD, a negative chest X-ray within 3 months is required
• Annual physical/health statement stating clearance to work (must be completed within 60 days of start)

Drug Screening

• Minimum 10-panel drug screen including THC within 60 days of start
• For California locations, additional saliva THC test required

Additional Notes

• Name verification affidavit required if there are discrepancies between credentials (optional)
• Certificate of Analysis (COA) not required for per-diem assignments

Prior Travel Experience
