Job Details

• Employment Type: Long-Term Assignment (LTA)
• Contract Length: 13 weeks
• Shift: Monday-Thursday 7:00 AM - 2:30 PM, Friday 7:00 AM - 3:30 PM (36 hours/week)
• Experience: Epic system experience preferred

Required Qualifications

• Minimum of 1 year of experience as a Histology Technologist, including experience within the last year
• Completion of Trinity Specialty Specific Competency Testing
• Successful completion of Core Exam
• Experience with Epic electronic health record system

Required Credentials and Compliance (Must be completed prior to or at onboarding)

• Fit Test
• Signed Code of Conduct form
• Completed I9 verification
• Confidentiality and Information System Security Agreement
• Integrity and Audit Services Form (signed shortly before start)
• Non-Employee Upload Form (pages 1-3 and 28)
• Color Vision test
• COVID-19 vaccination proof (including booster by Sept 2025), or approved declination/exemption (declination not accepted in CA or IL)
• MMR vaccination proof (2 doses or IgG titer lab report)
• Hepatitis B vaccination series started prior to start date (titers not accepted)
• Influenza vaccine proof or declination/exemption (optional)
• Tuberculosis screening: 2-step PPD or IGRA test; if positive, chest X-ray per regional requirements
• Varicella vaccination proof (1 dose or IgG titer lab report)
• Physical/Health Statement confirming fitness to work (exam within last year)
• 10-panel drug screen completed within 30 days of start (no THC) with Medical Review Officer signature
• Resume and skills checklist (within one year) plus two professional references
• Conflict of Interest Disclosure form
• Criminal background check (federal, nationwide, county for last 7 years) including SSN verification, dated within 30 days of start
• Education verification (highest academic degree or diploma/GED)
• Exclusion and sanctions checks (EPLS, OIG, OFAC) dated within 30 days of start
• National Sex Offender Registry check within 30 days of start
• System Access Form completed and uploaded one week before start
• Skills checklist provided by agency for clinical contractors
• Current Tdap vaccine or declination (declination accepted if no pediatric interaction)
• Child and Adult Dependent Abuse Training (Iowa only)
• Iowa-specific criminal history, sex offender, and abuse registry checks within 30 days prior to start

Additional Information

• Prior travel experience is not specified as required for this position.
• This role requires working primarily early morning to mid-afternoon shifts Monday through Friday with a shorter Friday shift.
• Epic electronic health record experience is preferred.
