• Scope: Operating Room surgical tech proficient in ortho, neuro, general surgery, robotics, urology, and vascular procedures

Required Credentials & Compliance

Certifications (Required for Onboarding)

• BLS: AHA or ARC only
• ACLS: AHA only

Licenses

• Professional License/Certificate Verification: Must be active and verified within 30 days of start; must be valid for the State of Louisiana

Skills & Competency Testing (Required for Onboarding)

• FMOLHS Skills Checklist: Unit-specific, updated annually within one year prior to start
• FMOLHS Competency Exam: Passing score of 80% or higher within one year prior to start
• FMOLHS Core Competencies: Includes HIPAA, Patient Bill of Rights, Safety Orientation, Bloodborne Pathogens, Fire Safety, Infection Control, Elder Abuse and Neglect, Age Specific Care, Cultural Diversity, Advance Directives; updated annually with passing score of 80% or higher

Personal Health Requirements (Required for Onboarding)

• MMR: Proof of 2 immunizations, positive titer, or declination with signed statement
• Fit Test: N95 mask fit test (3M 1860, 1860s, or 1870+), completed within 1 year prior to start, updated annually
• Hepatitis B: Proof of 3 immunizations, positive titer, or declination with required signatures
• Influenza Vaccine: Required during flu season (Oct 1 - Apr 30); medical or religious exemptions accepted with proper documentation and approvals
• TB Test: Negative QuantiFERON blood test within 1 year or 2-step skin test; if positive, must provide clearance or treatment proof
• Health Statement: Signed and dated within 1 year confirming ability to work without restrictions
• Varicella: Proof of 2 immunizations, positive titer, or declination with signed statement
• Tdap: Proof of vaccination within past 10 years or declination with required signatures
• Annual TB Questionnaire: Required within 6 months of start; if positive, documentation of evaluation and treatment completion required
• Eye Exam & Color Vision: Ishihara Color Test and Snellen Vision test within 1 year prior to start

• Criminal Background Check: Must be completed within 30 days prior to start, including SSN trace, county and national checks, education and employment verification (last 5 years), driving record, and multiple federal databases
• Drug Screen: Negative 10-panel drug screen within 30 days prior to start
• HRIS Form: Required for submission
• Badge Photo: Forward-facing, well-lit JPEG with neutral background
• Annual Education Packet: Required for onboarding
