Licensure

• Must have Kentucky or compact nursing license valid for Kentucky

Certifications Required

• BLS (AHA only, front and back of certificate if not electronic)
• ACLS (AHA only, front and back of certificate if not electronic)
• NIH Certification (required for RN specialties except OR)

Electronic Medical Record System

• Meditech EMR experience preferred

Orientation

• Bi-weekly orientation sessions
• Start date on job is the start date needed

Candidate Requirements

• Must reside within 100 miles of the facility to be considered
• Driver’s license required for submission
• Only submit candidate to one job at a time

Health & Immunization Requirements

• COVID-19 Vaccine (proof of completed series; religious and medical exemptions accepted with documentation)
• MMR: Proof of 2 vaccinations or positive titer required prior to start
• Hepatitis B: Positive titer or proof of 3 vaccine series or declination accepted
• Varicella: Proof of 2 vaccinations or positive titer required prior to start
• Tdap: Valid within 10 years; declinations accepted
• Influenza Vaccine: Required October-March; exemptions require approval
• TB Quantiferon Blood Test: Within one year prior to start; annual update required
• Annual Physical or Physician note within 1 year prior to start stating clear to work without restrictions

Background & Compliance

• Background check within 30 days prior to start (7-year county, nationwide criminal, SSN trace, sex offender, SAM, OIG)
• Drug screen within 30 days prior to start (includes multiple substances; marijuana/THC excluded)
• Education and employment verifications required
• Kentucky Vulnerable Adult Maltreatment Registry verification within 30 days prior to start
• Social Security Trace, OIG Search, EPLS, OFAC, SAM, National Sex Offender checks as applicable

Skills & Competency

• Unit-specific skills checklist within one year prior to start; renewed annually
• Competency exams (including EKG and Telemetry) with passing score of 80% or higher within one year prior to start
• Core competencies including HIPAA, Patient Bill of Rights, Safety Orientation, Bloodborne Pathogens, Fire Safety, Infection Control, Elder Abuse and Neglect, Age Specific Care, Cultural Diversity, Advance Directives; renewed annually

References

• Two professional references required: one charge nurse or higher, one supervisor reference
