Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) – Long Term Care (LTC) – Nights

Location: Bellaire, Michigan

Hours: 12 hours per shift, approximately 36 hours per week (every other weekend plus 2 weekday shifts)
Contract Length: 13 weeks

Job Summary

Provide nursing care in a long-term care setting by administering medications and treatments, assisting physicians during exams, monitoring residents’ conditions, and coordinating nursing services on the unit. Maintain accurate documentation including admissions, discharges, transfers, incident reports, and narcotics records. Participate in care planning, nursing assessments, and facility audits. Maintain resident confidentiality and comply with all Resident Rights rules.

Required Qualifications

• Valid LPN / LVN license in Michigan
• Minimum 2 years of experience in Long Term Care, including Skilled Nursing experience
• Long Term Care experience within the last year
• Completion of at least one prior successful travel assignment as an LPN/LVN
• BLS certification issued by the American Heart Association

• Provide work history with explanations for any employment gaps
• Submit a skills checklist specific to the discipline (dated within the last year)
• Provide two supervisory references from the past two years
• COVID vaccination status must be disclosed at submission
• Must be flexible with scheduling; block scheduling is not approved
• Locals residing within 50 miles of the facility are not eligible for this assignment

• Facility operates on a Sunday-Saturday schedule; weekend shifts required
• Limited housing options available; candidates should arrange housing in advance

Responsibilities

• Administer prescribed medications and treatments per nursing standards and facility policies
• Assist physicians during treatments and examinations
• Monitor and document residents’ vital signs and reactions to treatments
• Coordinate nursing services and assign duties on the resident unit
• Participate in orientation, training, and evaluation of nursing personnel
• Maintain unit narcotics records and ensure cleanliness and comfort of resident rooms
• Complete nursing assessments, care plans, and special projects as assigned
• Report complaints, unusual problems, accidents, and injuries promptly

Scheduling & Hours

• 36 hours per week: every other weekend plus two weekday shifts
• Shifts are 12 hours each, primarily nights
• Scheduling flexibility required; no block scheduling allowed

• Timesheets are not signed by the facility and will be uploaded unsigned for review
• Overtime, holiday, and callback pay rates apply but are not disclosed here
• Agency must have an active Michigan iCHAT account and be authorized to schedule fingerprinting
