Schedule
- Day shift, 5x8 hours
- Monday–Friday, with some flexibility
- 40 guaranteed hours per week
Assignment Details
- Travel/contract role
- Home health setting
- Primarily geriatric population
- Daily point expectation: 7–8
- Phone interview
- Locals accepted at the same rate
- Charting system: Perfect Notefield (training provided)
Required Qualifications
- Valid Occupational Therapist license in California
- ACLS issued by American Heart Association or American Red Cross
- BLS issued by American Heart Association or American Red Cross
- PALS issued by American Heart Association or American Red Cross
Required Documents and Health Requirements
- Government-issued photo ID
- Emergency contact form before start
- Personal or auto insurance
- Physical exam within 1 year of start
- Negative TB test or chest X-ray within 1 year of start
- Drug screen within 1 year of start
- Flu vaccination or declination during flu season
- Proof of immunity or vaccination documentation for measles, mumps, rubella, hepatitis B, and varicella, as specified by birth year and test/vaccine history
- Background check within 1 year of start
- EPLS/GSA/SAM screening annually
- HHS/OIG screening annually
- Skills checklist annually
