Job Overview

• Travel/contract physical therapy role in home health
• Day shift, 5x8 schedule
• 40 hours per week
• 13-week assignment

Work Setting

• Home visits
• Periodic office meetings
• May also visit patients in care facilities
• Home health and hospice setting
• Territory includes travel up to 75 miles from the home clinic
• Services cover Vance and Warren Counties

Required Qualifications

• Valid Physical Therapist license in North Carolina
• Physical Therapist experience within the last year
• One completed travel assignment
• BLS issued by American Heart Association or American Red Cross
• Experience working with adult patients
• Experience in a home health setting
• Driver's license
• Experience with Homecare Homebase

Schedule Notes

• Weekend schedule varies by location and profession
• No guaranteed hours
• No on-call requirement listed
• Some locations may use a Baylor shift schedule of Friday through Monday, 4x10s
• Most assignments use standard Monday through Friday 5x8s

Productivity Expectations

• Average 5–6 patients per day
• 30 points per week expected

Additional Notes

• Local candidates are accepted
• Mileage is reimbursed at the IRS standard rate
• Travel time is compensated from patient home to patient home
• Commute to the first patient and from the last patient is not billable
