Job Overview

Location: Portland, Oregon
Employment type: Travel/Contract
Shift: Day shift, 5x8 hours, 8:00 AM–5:00 PM
Contract length: 13 weeks

Setting and Caseload

Home health role with home visits and periodic office meetings
Average patient load: 5–6 patients per day
Typical assignment includes start-of-care and follow-up visits
Territory may require travel up to 75 miles from the home clinic

Required Qualifications

Valid Physical Therapist license in Oregon
Physical Therapist (1 year of experience)
Physical Therapist experience within the last year
One completed travel assignment
BLS issued by American Heart Association or American Red Cross
Experience in a Home Health setting
Driver's License
