Required Credentials & Certifications

• California SLP License (mandatory)
• BLS (Basic Life Support): State mandate for RNs and LVNs (AHA or ARC only)
• CPI (Crisis Prevention Institute): Required for all behavioral staff; preferred to complete prior to start (can be completed through the district, but no overtime pay for training through district)
• Educational Services - CA Positive Behavior Training Certificate: State mandate, must be completed yearly prior to start
• Educational Services - Mandatory Reporter Training: Required prior to start; must score 80% or higher and upload certificate (valid for 1 year)
• Harassment Prevention Training Certificate: Required for all candidates
• Fingerprinting:

  • Facility does not require their own fingerprints; Aya fingerprint attestation must be sent to client prior to start
  • California DOJ FBI/BCI Fingerprint Result Attestation required (state mandate)
  • Live Scan or FD-258 fingerprint card required depending on residency status
• State License Verification: Must be verified twice (initial and 30 days prior to start)
• NURSYS Verification: Required by Aya for nursing staff (if applicable)

Background Checks & Compliance

• Criminal background check within 1 year of start and updated annually
• EPLS/GSA/SAM and HHS/OIG checks required within 1 year of start and updated annually
• Transcript/Diploma verification as required by the school
• Fingerprint clearance attestation required prior to start
• Proof of name change if applicable
• Emergency contact form and other standard employment documents required

Health & Safety Requirements

• TB Risk Assessment: California state mandate for all candidates, including remote; must be completed within 60 days of hire or have valid documentation within 4 years
• Negative TB or Chest X-Ray: Required if applicable, with specific rules for past positive TB results
• Physical Exam: Not required by Aya unless requested by client
• Drug Testing: Not required unless specifically requested by client
• Vaccinations (Flu, COVID-19, etc.): Not required by Aya unless requested by client or state mandate

Additional Notes

• Orientation Safety Manual Acknowledgement required annually
• Skills Checklist required for clinical staff (including SLPs)
• Educational Services State Teaching Credential and ASHA Certification are not required for this SLP position at this facility
• Paraprofessionals require a high school diploma or equivalent
• Audiometry Certificate may be required for RNs only (not applicable for SLPs)
