Job Opportunity: Occupational Therapist - Home Health (Travel/Contract)

Location: Burlingame, California (San Francisco metropolitan area, majority downtown)

Job Description

  • Patient Population: Patients with alcohol or substance abuse issues (no direct work with homeless population, but must be comfortable working in an area with a high homeless population)
  • Schedule: 4/10s or 5/8s
  • Patient Load: 5-6 patients per day
  • Points Requirement: Minimum 26-30 points per day/week to qualify for 40 guaranteed hours
  • Guaranteed Hours: 40 hours per week if points requirement is met and timecard is approved
  • Types of Cases: Home Health focusing on gait, strength, and range of motion
  • Experience Required: Minimum 1 year of Home Health/OASIS experience
  • First-timers Accepted: Yes
  • On-call: No
  • Local Candidates: Accepted within a 50-mile radius
  • Mileage Reimbursement: IRS standard
  • Licensing: Must have a valid California OT license at time of submission (no pending licenses accepted)
  • Special Requirements: References from previous Home Health or Hospice agencies required

Required Certifications & Documents

  • Certifications:
    • BLS (Basic Life Support) required at time of submission
    • ACLS and PALS preferred (American Heart Association or American Red Cross)
  • Licenses: Valid California OT license required
  • Background Checks: Standard 7-year search including SS trace, county searches, nationwide, sex offender registry
  • Health Documents:
    • Physical exam within 1 year of start
    • Negative TB test or chest X-ray within 1 year
    • Immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella
    • Drug screening (9 panel) within 1 year
    • Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31)
  • References:
    • 2 professional references for travel, allied, or per diem acute care
    • 1 professional reference for per diem non-acute care
  • Competency Exams: Medication test/pharmacology and other competency exams as applicable
  • Other: Emergency contact form required prior to start

Additional Notes

  • Must be comfortable navigating and parking in a metropolitan area with a high homeless population (though not working directly with this population).
  • Local candidates within 50 miles are accepted.
  • Must have client approval on timecards documenting visits and points.
