Job Description

• Position: Clinical Oncology Pharmacist
• Shift: Monday to Friday, variable between 7 AM and 6 PM
• Weekend shifts: Required as per clinic needs
• On-call: Not required
• Floating: Not required

Requirements/Qualifications

• Valid Pharmacist license in California
• Minimum 2 years of Pharmacist experience, including Oncology Clinical Infusion
• Pharmacist experience within the last year
• Experience with Epic electronic health record system

Licensure

• Must hold a current California Pharmacist license

Required Qualifications & Certifications

• Current and valid Driver’s License or government-issued photo ID
• Background checks including Social Security Trace, 7-year criminal history, sex offender search, and federal exclusions (must be within 30 days of start)
• Education verification (diploma or official transcripts)
• Advanced Cardiac Life Support (ACLS) certification (AHA only)
• Basic Life Support (BLS) certification (AHA or ARC)
• Chemotherapy certification (required for Oncology roles)
• Other certifications may be required depending on unit or specialty

Health & Compliance Requirements

• Physical exam within 1 year of start, updated annually
• Fit mask testing within 1 year of start (specific mask models required)
• Immunizations or proof of immunity for:

  • Tetanus/Diphtheria/Tdap (declination accepted)
  • Negative TB test or chest X-ray within 1 year
  • Measles (Rubeola), Mumps, Rubella (positive titer or vaccination proof)
  • Hepatitis B (titer, declination, or completed vaccine series)
  • Varicella (positive titer or vaccination proof)
• Drug screening: 9-panel drug screen within 12 months of start
• Seasonal flu vaccination or signed declination (required during flu season)
• COVID-19 vaccine or declination (specific vaccine timing and doses required)

Documentation & Onboarding

• References: 2 required, supervisor level or above, one within the last year
• Employment application covering last 7 years with explanations for gaps over 30 days
• Work history verification for last 5 years
• Skills checklist and other facility-specific onboarding documents required
• Completion of facility-specific training modules including HIPAA, harassment prevention, and others prior to start
• Photo ID for badge issuance required within 72 hours of signing

Additional Notes

• Floating to sister facilities is not required
• On-call duties are not required
• Candidate must complete all required background checks and certifications prior to start
• Facility-specific compliance and training modules must be completed and documented as part of onboarding
