Job Description

• 1 year of transplant experience required
• Inpatient pharmacy experience required
• Experience with EPIC electronic health record system required
• Weekend rotation required
• Board Certified Pharmacist (PharmD) required
• Must have Louisiana pharmacist license in hand at time of submission

Required Qualifications

• Valid Clinical Pharmacist license in Louisiana
• Board Certified PharmD
• Minimum of 1 year of clinical pharmacist experience, including experience within the last year
• Experience in an inpatient setting
• Proficiency with Epic electronic health record system

Compliance & Documentation Requirements

• Government-issued photo ID matching candidate documentation
• Background check within 6 months covering all locations lived/worked/schooled in past 7 years, including social security trace, nationwide and federal checks, sex offender registry, and others; must be updated annually
• Employment eligibility verification (E-Verify)
• Drug screening within 30 days of start date (10-panel or expanded panel depending on hire date)
• Physical exam within 12 months of start (not required by employer but required by placement agency)
• Immunizations and health screenings including:

  • Tetanus/Diphtheria/Tdap (within 10 years)
  • TB testing (two-step skin test or equivalent within 3 months of start)
  • MMR (Measles, Mumps, Rubella) vaccination or positive titer
  • Hepatitis B series, titer, or declination
  • Varicella vaccination or positive titer
  • Seasonal flu vaccination/documentation (required Oct 1 – March 31) with exemptions accepted
• Annual competency exams including medication/pharmacology competency with minimum 80% score
• Compliance with employee appearance policy and other facility-specific policies

Certifications (may be required depending on unit)

• ACLS (Advanced Cardiac Life Support)
• BLS (Basic Life Support)
• Other certifications as applicable to specific units or roles

Additional Notes

• Candidates must provide 7 years of work history with no gaps longer than 30 days
• Prior travel experience is not explicitly required for this role
• Weekend rotation is part of the schedule

This position is ideal for a licensed inpatient pharmacist with transplant experience who is comfortable working in a hospital setting with EPIC and is available for a 13-week travel contract including weekends.
