Job Details

Required Background and Compliance Documents

- State-Specific Searches (e.g., WA State Patrol Search, OBRA Registry for CNAs in WA)
- Providence-specific floating policy acknowledgment (may require floating to other Providence facilities)
Health and Safety Requirements
- Physical Exam (within 1 year of start)
- Fit Mask Test (annual, travel only)
- Tetanus/Diphtheria/Tdap Vaccine (within 10 years or declination)
- Negative TB Test or Chest X-Ray (annual update required)
- MMR Vaccines or Positive Titers for Rubeola, Mumps, Rubella
- Hepatitis B Vaccination or Declination
- Varicella Vaccination or Positive Titer
- 10 Panel Drug Screen (within 90 days of start)
- Seasonal Flu Vaccine or Declination (required during flu season)
- COVID Vaccine or Declination
- Respiratory Safety Clearance (annual for travel candidates)

Competency and Skills Requirements

- Medication Aide Exam (for CNAs in LTC Oregon Assisted Living facilities)
Training and Modules
- Completion of Providence WorkRamp Core and Specialty Modules as assigned based on region, profession, and specialty
- Epic Access Training Modules (required for clinical staff in certain regions)
- Annual Regulatory Modules (system-wide compliance training)
References and Employment History
- Minimum of 2 supervisor references covering at least 1 year of employment within the last 3 years in a comparable specialty/setting
- Employment application covering at least the last 5 years (7 years for APN/Subcontractors) with month/year format for dates worked
- Clarification required for any gaps or overlaps in employment
Additional Notes
- Floating between units is required
- First-time travelers are accepted
- Pending licenses are not accepted
- Background checks and certifications must be current and compliant with Providence Health and Services policies
- Compliance with all Providence-specific policies and training modules is mandatory
