Job Opportunity: LVN / LPN - Travel/Contract
Location: Sacramento, California
Contract Length: 13 weeks
Job Description
- Provide routine patient care, support, and related non-professional household services to meet hygiene, activities of daily living, and comfort needs of homebound patients.
- Work within legal scope of practice and established standards of care, policies, and procedures.
- Integrate education, technical competence, and patient interaction skills to gain patient and family cooperation.
- Exercise sound judgment and clinical skills to support patient care plans based on diagnosis.
- Adhere to all local, state, and federal regulations to ensure privacy and safety.
- May perform specific procedures and teaching duties.
- Floating to other units or campuses required; Home Health Aide (HHA) duties include providing home care services at participants' homes.
- Weekend work may be required (approximately one weekend per month).
- No on-call required.
Required Certifications & Licensure
- Valid California LPN/LCN License
- Home Health Aide (HHA) certification
- BLS (Basic Life Support) Healthcare Provider certification
- Valid Driver’s License and Auto Insurance (required due to home care visits)
Experience Requirements

Background Checks & Compliance

Health & Immunization Requirements
- Physical exam within 1 year of start (must indicate cleared for work).
- Fit mask test within 1 year, renewed annually (exceptions apply).
- Adult Tetanus/Diphtheria/Tdap vaccination within 10 years or lifetime TDAP with booster within 10 years.
- Negative TB test (QGold or TSPOT within 90 days) or 2-step PPD required, renewed annually.
- Measles (Rubeola), Mumps, Rubella: Positive IGG titer or vaccination series required; boosters required if titers negative or equivocal.
- Hepatitis B: Positive titer, vaccination series, or signed declination required.
- Varicella: Positive IGG titer or vaccination series required; boosters if titers negative or equivocal.
- Seasonal Flu vaccination required during flu season (Nov 1 - Apr 30), with signed declination accepted.
- Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids due to CA law).
Additional Requirements
- Unit-specific skills checklist required, renewed annually.
- Full 7 years of work history required on application with no gaps; employment verification up to 7 years required.
- References: 2 professional references required for clinical candidates (Aya requirement, not facility).
- Candidate must review and sign facility-specific documents including job description, standards of business conduct, code of conduct, vendor training, overpayment refund policy, and regulatory orientation prior to start.
- TB screening questionnaire required within 90 days of start and renewed annually.
- Personal or auto insurance must be effective for the duration of the assignment due to driving requirements.
Work Environment & Other Notes
- Floating to other units or campuses is expected.
- Home visits required as part of HHA duties.
