Facility Setting

Small acute care hospital with a 4-room Operating Room (OR)

Shift Details

• Rotating shifts between 7:00 AM - 3:30 PM and 10:00 AM - 6:30 PM
• 40 hours per week (5 days x 8 hours)

Required Certifications

• CBSPD (Certified Biological Safety Processing Technician)
• BLS (Basic Life Support) - AHA or ARC only

Required Background Checks and Verifications

• 7-year background check within 6 months of start, including SSN trace with all aliases, county searches, nationwide sex offender search, education verification (not required if highest education is high school), and international search if applicable
• FACIS Level 3 screening within 2 weeks of start and monthly thereafter (or equivalent checks if FACIS III cannot be run)
• EPLS/GSA/SAM, HHS/OIG, OFAC Sanctions List, Medi-Cal Suspended and Ineligible Provider List as applicable
• Sex Offender Search within 90 days of start
• Education verification with official transcripts/diplomas if needed
• Employment history verification for up to 7 years with no gaps over 30 days
• References: 2 professional references required (Aya requirement only)
• Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids due to CA law)
• Physical exam within 1 year of start, including clearance for work and any work restrictions documented
• TB screening questionnaire within 90 days of start, renewed annually, with negative TB test or chest x-ray as required
• Immunizations and titers:

  • Tetanus/Diphtheria/Tdap within 10 years
  • Measles (Rubeola), Mumps, Rubella, Varicella, Hepatitis B (with declination option)
• Seasonal flu vaccination or signed declination during flu season (Nov 1 - Apr 30)
• Fit mask test within 1 year of start, renewed annually (with accommodations for religious exemptions or failed tests)
