Job Opportunity: Ambulatory Care LVN / LPN
Location: Worcester, Massachusetts
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Details
- Shift: Day shift, 5 days per week, 8 hours per day (8:00 AM – 4:30 PM)
- Work Setting: Hospital-based clinic covering outpatient services and inpatient consults
- Charting System: Epic (experience preferred)
- Weekend Requirement: No weekends required
- Start Date: Must be able to start on 7/8 (no exceptions)
- Preference: Candidates who have worked in the system within the last 12 months are preferred
Required Qualifications
- Valid LPN / LVN license in Massachusetts
- Minimum 2 years of LPN / LVN experience, including experience within the last year
- One completed travel assignment
- Basic Life Support (BLS) certification issued by the American Heart Association
- Ambulatory Care experience
Compliance and Documentation Requirements
- Valid government-issued ID (e.g., driver’s license or state ID)
- Massachusetts Statewide iCORI Release (time sensitive)
- 7-year criminal background check within 30 days of start, including SSN trace, county searches, national criminal search, and federal exclusions
- Education verification of highest completed education (transcript acceptable if verification unavailable)
- Required health documents within specified timeframes:
- Physical exam within 12 months, including physician statement of fitness without restrictions
- Negative TB test or chest X-ray as applicable
- Immunizations or declinations for: Tetanus/Diphtheria/Tdap, MMR, Hepatitis B, Varicella, Rubeola (Measles), Mumps, Rubella
- Drug testing within 30 days of start, covering a comprehensive panel of substances
- Seasonal flu vaccination or facility-specific declination (required during flu season)
- COVID-19 vaccination (most recent) or facility-specific declination
- Pass required medication competency exam prior to or on first day of orientation
- References: Two references within the last three years
- Employment application documenting 7 years of employment history with explanations for gaps over 30 days
- Skills checklist completed prior to start and updated annually as applicable
Additional Notes
- Candidates must pass medication exams with a minimum score of 80% on the first day of orientation to avoid cancellation
- Background check and compliance attestations must be completed and uploaded as required
- Some declinations (e.g., MMR, TDAP, Varicella) are not accepted for Pediatrics, Maternity, or NICU areas (note: this position is ambulatory care, so declinations may be accepted)
- Candidates with positive TB results must provide a chest X-ray and a physician release letter
- Facility uses Epic for charting; experience preferred but not mandatory
