PT Home Health - Travel/Contract Opportunity

Location: San Ramon, California (Service Areas include Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City, Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont)
Contract Length: 13 weeks
Shift: Day shift, 8 hours per day (8:00 AM – 4:30 PM), 5 days per week
Hours: 40 hours per week (guaranteed 36 hours weekly after ramp-up period)

Job Description

• Provide physical therapy services in a home health setting.
• Patient load: 5-6 patients per day, with a 1:1 patient-to-therapist ratio.
• Weekend patient visits are optional to meet productivity goals.
• No on-call responsibilities.
• Must have reliable personal transportation; occasional travel radius may exceed 50 miles.
• Documentation and timely communication are required; documentation tools will be provided.
• The system groups clients to minimize drive and windshield time.

Required Qualifications

• Valid Physical Therapist license in California.
• Minimum 1 year of Physical Therapist experience, including experience within the last year.
• Experience in a Home Health setting.
• Completion of at least one travel assignment.
• Basic Life Support (BLS) certification (American Heart Association or American Red Cross).
• Must have own transportation.

Compliance & Onboarding Requirements

• Government-issued photo ID (Driver’s License or State ID).
• Emergency contact information form.
• I9 attestation and standard background checks (7-year search including SS trace, county, nationwide, sex offender).
• Annual EPLS/GSA/SAM and HHS/OIG checks.
• Physical exam within 1 year confirming clearance to work without restrictions.
• Immunizations and health documentation including:

  • Tdap (within 10 years or annual declination)
  • Negative TB test or chest X-ray within 1 year
  • Measles, Mumps, Rubella (2 vaccines or positive titers within required timeframes)
  • Hepatitis B (titer, vaccine series, or declination)
  • Varicella (2 vaccines or positive titer)
  • Seasonal flu vaccine or declination (required year-round)
  • COVID-19 vaccine documentation or facility-specific declination form
• 10-panel drug screen within 1 year of start.
• Auto insurance and registration documentation (must be current throughout assignment).
• Facility-specific forms including arbitration agreement, confidentiality agreement, and employee data forms must be completed prior to start.

• Mileage reimbursement at IRS standard rates.
• Training provided for documentation systems (MediScrib & OASIS).

• 2 professional references required for travel, allied, or per diem acute care roles (within 3 years).
• 1 professional reference required for per diem non-acute care roles.
• 1 year of employment verification related to the profession is required.
