Hospice Registered Nurse (RN) - Travel/Contract

Location: Kyle, Texas
Contract Length: 13 weeks

Job Overview

• Provide skilled nursing care to hospice patients in their homes.
• Perform comprehensive patient assessments and collaborate with interdisciplinary teams to develop and oversee individualized Plans of Care.
• Responsible for admissions and must complete charting on-site using Homecare Homebase (HCHB) and OASIS systems.
• Average caseload: 4-6 patients per day.
• Work schedule: Monday to Friday, 8:00 AM to 5:00 PM, with weekend shifts as needed and participation in an on-call rotation (one weeknight call required).
• Must be willing to travel up to 50 miles from home base to see patients.

Required Qualifications

• Valid Registered Nurse (RN) license in Texas.
• Minimum 1 year of hospice nursing experience within the last year.
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
• ACLS certification.
• BLS certification issued by the American Heart Association or American Red Cross.

Work Environment & Expectations

• In-home patient care with a geriatric population.
• Interdisciplinary care coordination and plan of care development.
• Dress code: Scrubs, any color.
• Must participate in on-call rotation and be available for weekend shifts as needed.

Compliance & Documentation Requirements

• Current Driver’s License/State ID required.
• Background checks including 7 years residency/work history, SS trace, county searches, sex offender registry, FACIS Level 1, state driver’s record, and federal exclusions (HHS/OIG, EPLS/GSA/SAM).
• Physical exam within 12 months confirming good health.
• Negative TB test or chest X-ray within required timeframes; annual TB questionnaire.
• Immunization or titer proof for Measles, Mumps, Rubella, Varicella, and Hepatitis B declination if applicable.
• 9-panel drug screen within 12 months.
• Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31).
• Annual skills checklist and competency exams including medication/pharmacology and infection control/bloodborne pathogens.
• Employment application and professional references required.

Additional Notes

• Overtime must be pre-approved.
• Mileage reimbursed at IRS standard rates.
• Locals within a 50-mile radius are accepted.
• First-time travel nurses are welcome to apply.

This position offers a full-time schedule with guaranteed minimum hours and the opportunity to work in a supportive home health and hospice environment.
