• No call
• 60-mile travel radius
• Estimated 6 patients/day

Contract Length

13 weeks

Required Qualifications

• Valid Physical Therapist license in Michigan
• Bachelor's or master's degree in physical therapy
• Experience in Supervisory roles
• Driver's License

Experience / Skills

• Knowledge of physical therapy principles, concepts, and methodology appropriate for terminally ill patients
• Problem-solving and decision-making ability
• Ability to manage patient care with knowledge and experience in bedside nursing, symptom management, crisis intervention, and family intervention
• Excellent communication skills
• Ability to develop and maintain rapport with patients, families, and staff
• One year or more of physical therapy work experience in a multidisciplinary setting is preferred

Key Responsibilities

• Develop and implement patient physical therapy plans of care
• Complete assessments, evaluations, and OASIS assessments
• Document care and maintain patient medical records
• Educate patients, families, and team members
• Participate in interdisciplinary team meetings and quality improvement activities
• Supervise Physical Therapist Assistants, Certified Nursing Aides, and Home Health Aides as needed
