Job Summary

Provide competent clinical nursing care consistent with professional standards in a home health setting. Responsible for coordinating and delivering patient care using the nursing process.

Key Responsibilities

• Assess patient/family readiness and developmental level for learning; use appropriate patient education materials.
• Identify expected patient outcomes and participate in revising plans of care as needed.
• Collaborate with interdisciplinary team members to evaluate and communicate patient care information.
• Deliver care to patients with complex problems, addressing physical, psychosocial, educational, and safety needs.
• Perform bedside point-of-care testing as required.
• Organize and prioritize care based on patient and family needs.
• Consult timely with resources for patients with complex care issues or high discharge risk.
• Participate in unit goals, performance improvement, and promote cost-effective care.
• Promote a safe, clean, and secure environment.
• Recognize and synthesize complex assessment data to determine care priorities.
• Collaborate with patients and families to develop and update care plans, addressing cultural and ethnic considerations.
• Engage in professional growth activities and intra/interdepartmental collaboration.
• Stay informed on public policy and regulatory guidelines affecting healthcare.
• Read relevant research and contribute ideas for practice improvement.

Required Qualifications

• Valid Registered Nurse (RN) license for North Carolina.
• Experience in nursing with ability to manage complex patient care in home health.
• Strong communication and organizational skills.
• Ability to work collaboratively within a multidisciplinary team.
