Job Summary

The RN Care Manager II collaborates with physicians and multidisciplinary team members to develop and monitor individualized patient care plans from admission through discharge. The role focuses on ensuring patients progress toward desired outcomes by continuous assessment and evaluation, coordinating care efforts, and resolving barriers to effective and timely patient care.

Key Responsibilities

• Develop and implement patient care plans, ensuring effectiveness and appropriateness of services.
• Coordinate and facilitate patient care progression across the continuum.
• Identify and resolve barriers that delay or hinder patient care and discharge.
• Collaborate with medical, nursing, and ancillary staff to promote efficient care delivery.
• Utilize conflict resolution skills to address and resolve issues promptly.
• Meet expectations of leadership competencies relevant to the role.

Requirements

• Registered Nurse with Associate Degree in Nursing or higher.
• Must provide proof of education and Nursys license verification, including any license discipline documentation.
• Prior travel experience is not specified or required.
